About the Project
50 Shades of Pink is a community wellness and documentary project created by AntFarm Wellness, LLC to honor the full spectrum of the breast cancer journey — from diagnosis to survivorship, caregiving to advocacy.
We believe every woman's story deserves to be told. This project creates a safe, affirming space for survivors, supporters, and community members to share their experiences, connect with others, and reclaim their narratives.
Participants may be featured in documentary content, wellness programming, and community events. All participation is voluntary and guided by your comfort level.
Who Can Participate
Breast Cancer Survivors
Women who have experienced a breast cancer diagnosis at any stage and are ready to share their journey.
Caregivers & Supporters
Family members, friends, and advocates who have supported someone through the breast cancer experience.
Community Advocates
Women passionate about breast cancer awareness, community wellness, and empowering others through storytelling.

Voluntary Participation
Your participation in 50 Shades of Pink is entirely voluntary. You may withdraw at any time without penalty or obligation.
Content Use
By participating, you grant AntFarm Wellness, LLC a non-exclusive license to use your story, image, and likeness in documentary content, wellness programming, and promotional materials related to this project.
Confidentiality
Personal health information shared during intake or programming will be kept confidential and will not be shared with third parties without your explicit consent.
Non-Clinical Nature
50 Shades of Pink is a wellness and community storytelling project. It is not therapy, medical treatment, or clinical care. Participants are encouraged to maintain their existing healthcare relationships.
Right to Review
Participants will have the opportunity to review any content featuring them before publication. You retain the right to request edits or removal of your content at any time.
Intellectual Property
All project content, branding, and programming materials are the intellectual property of AntFarm Wellness, LLC. Participants agree not to reproduce or distribute project materials without written permission.
Media Release Notice
By completing this intake form, you acknowledge that AntFarm Wellness, LLC may photograph, video record, or audio record your participation in 50 Shades of Pink programming and events.
This media may be used for documentary content, social media, website features, wellness education materials, and community outreach — all in connection with the 50 Shades of Pink project and AntFarm Wellness, LLC's mission.
You will not receive financial compensation for the use of your image or story. However, you retain the right to request that your content be removed from future use at any time by contacting us at hello@antfarmwellness.com.
Privacy Notice
AntFarm Wellness, LLC is committed to protecting your privacy. Information collected through this intake form is used solely to coordinate your participation in 50 Shades of Pink.
We do not sell, rent, or share your personal information with third parties for marketing purposes. Your data is stored securely and accessed only by authorized AntFarm Wellness team members.
